Every sponsor and camper must have a completed registration form upon arriving at camp.

Enrollment Forms

Churches, please print one of each form below and fill out for all boys and all girls attending (adults and youth). If you are bringing a larger group you can print multiple copies as needed. Please print these in color to preserve the pink and blue strip if possible.
